Group Living Arrangement Definition
A certified public or private nonprofit residential setting that serves no more than 16 blind persons or persons with a disability. The nonprofit is certified by the appropriate agency or agencies of the State under Section 1616(e) of the Social Security Act.
Homeless Definition
A homeless budgetary unit is defined as a budgetary unit in which all participants meet one of the following criteria:
They do not have a fixed or regular nighttime residence.
The participants have as their primary nighttime residence one of the following:
A supervised shelter designed to provide temporary shelter to homeless persons.
A half-way house or similar institution that provides temporary residence. When the institution provides meals, and the participant does not have the obligation to pay shelter costs, see Residents of an Institution.
A rent free accommodation in the residence of another person for not more than 90 days.
A place not designed, or ordinarily used, for sleeping. This includes, and is not limited to, the following:

Hospital Designated Representative Definition
A hospital designated representative is an individual who is acting on behalf of an incapacitated or deceased applicant.
NOTE This can be a hospital social worker or patient representative.
Hospital Patient Representative Definition
An employee of a hospital or an agency contracted by the hospital. This assists providers with the following services:
Offers information on the application process for AHCCCS Health Insurance.
Locates someone to apply on behalf of an incapacitated patient.
Assists an applicant with completing an AHCCCS application for health insurance.
Forwards the application to the designated DES staff or the servicing DES local office.
Assists DES in obtaining needed verification.
