Information Exchange – WMAT TANF
AZTECS and the White Mountain Apache Tribal (WMAT) TANF Program do not interface.
FAA and WMAT TANF staff use the Tribal TANF Turn-Around Document (TAD) (FAA‑1125A) to exchange information.

Exchange of information must be faxed or hand carried by close of business the following workday(g). The information is sent to the attention of the FAA Local Office Manager or the WMAT TANF Manager.

The FAA‑1125A is used for the following:
●To send all change information to either an FAA local office or the
WMAT TANF office.
NOTE When FAA receives change information from the WMAT TANF office, the FAA
‑1125A and change information is date stamped and processed from the date it is received in the FAA local office. (See

●To send FAA applications received or completed at a WMAT TANF office for FAA programs.
When the application is received by any of the following means, date stamp and process the FAA programs the date received in the FAA local office:
●Fax
●Hand carried
●Mailed in
