Illegal Drug Use Statement Provided by the Participant

When the completed Illegal Drug Use Statement (FAA-1415A) form or the Illegal Drug Use Statement (A002) notice is provided, review the responses to all questions on the statement. Responses must be reviewed, and all follow up actions completed, within two workdays(g) after receipt.
 
When all questions are answered and all responses are NO, complete all of the following:
 
Document the case file with the date the Illegal Drug Use Statement was received, and all responses were marked NO.
Complete the CA eligibility determination for the budgetary unit.
Send the appropriate notice.
 
NOTE When a CA approval notice was sent for a new CA application, send the correct change notice.
 
When any question is answered with a YES response, complete all of the following:
 
Refer the participant for a drug test. (For instructions on how to refer a participant, see Drug Test Referral Procedures.)
Document the case file with the date the Illegal Drug Use Statement was received and the date the participant was referred to Central Office for a drug test.
 
The participant remains coded as IN while the test and results are pending. When the results are received, take action based on one of the following:
 
When the participant tested positive for drug use, complete all of the following:
Document the case file with the results
Disqualify the participant
Complete the CA eligibility determination for the budgetary unit
Send the appropriate notice
When the participant tested negative for drug use, complete all of the following:
Document the case file with the results
Complete the CA eligibility determination for the budgetary unit
Send the appropriate notice
 
When any question is unanswered, consider the Illegal Drug Use Statement to be not provided by the participant. (See Illegal Drug Use Statement Not Provided by the Participant) for policy and procedures.)
