Death of a Parent

The participant may be eligible for other types of benefits when deprivation is due to the death of a parent.
 
Complete absent parent screens on deceased legal parents and natural (biological) parents.
 
When deprivation is due to the death of a parent, the death must be verified.
 
Request the PI provide verification of the death of the absent parent. Assist the PI to obtain verification. (See Providing Verification) The PI is responsible for any fees required to obtain the needed documents.
 
Documents that may verify the death of a parent may include, but are not limited to, the following:
 
A letter from an out-of-state vital statistics agency
Burial Payment records
Death Certificate
Hospital records
In Memoriam card
Indian census records
Institutional records
Insurance Company records
Minister statement
Newspaper obituaries
Police records
Public Benefits records
Receipt of benefits being received based on a deceased parent, such as SSA or VA
Signed funeral director's statement
VA or military service records
