CA PRA Immunization Requirements

CA participants must comply with the immunization requirements or establish Immunization Good Cause. (See Personal Responsibility Agreement)
 
Each dependent child is required to be immunized in accordance with the provisions of the Arizona School Child Care Immunization Law.
 
Review the official application at each new and renewal interview. Determine whether the Immunization question is completed for each child. The PI's signature on the application is acceptable verification.
 
This requirement may be waived. The parent or specified relative must either meet the immunization good cause requirements or provide documentary evidence that supports both of the following:
 
●They do not consent to immunization of the child, based on personal beliefs.
●They understand the risks and benefits of immunization.
