Policy
The participant and the authorized representative have the right to a printed copy of the Your Rights and Responsibilities (FAA-1573A) form.
The FAA-1573A is available to the public in the DES Documents Center. To print a copy of the Rights and Responsibilities, complete all of the following:
●Go to the DES website at des.az.gov.
●Click on Documents Center.
●Enter the complete form number, FAA-1573A, in the search box, and click “Search.”
●Click on the link in the “Download” column for the form to download a copy.
