Accept only the originals of the following documents to verify a participant's identity:
●Certified birth certificate issued by any of the following bureaus of vital statistics:
U.S. government
State government
Local government
●U.S. passport.
●Citizenship and immigration document.
●I.D. issued by a non-governmental health plan.
●Non-emergency AHCCCS I.D. card.
●Voter Registration card.
●Driver’s license or state issued I.D. from any of the following:
