State Assistance Eligibility and Benefit Determination - Tuberculosis Control (TC) Case Completion
When the designated local staff member receives the TB-108 from the LOM, the designated staff member completes one of the following:
●When the applicant is known to AZTECS, sends the S001 notice to the applicant.
●When the applicant is not known to AZTECS, mails an application to the applicant with an instruction to submit the completed application within 10 calendar days.
When an application is received from the applicant, complete both of the following:
●Register the application. The date of application is the date the application is received by the FAA local office.
●Schedule an interview.
When an application is not received from the applicant, complete the following:
●Complete the TB-108. Document that no application was received from the participant.
●Return one copy to DHS.
●Retain the second copy at the FAA local office.
Notify the Department of Health Services (DHS) of the decision reached on the TC application. Complete the section of the Request for Approval of Home Care for Tuberculosis Patient (TB‑108) form with the heading DEPARTMENT OF ECONOMIC SECURITY ACTION. Return one copy to DHS. Retain the second copy at the FAA local office.
