Nonfinancial Eligibility - Identity of the PI

The identity (ID) of the PI must be verified. Acceptable documents include the following:

●A government issued photo ID is the primary source of verification.

●Verification through an electronic verification source when the primary source is unavailable.

●A signed Disaster Nutrition Assistance Program Affidavit Attesting to Identity (FAA-1723A) form attesting to the ID of the participant. The person attesting to the PIs ID must provide a government issued photo ID.
