Information Exchange – SRPMIC

FAA and Salt River Pima-Maricopa Community (SRPMIC) LEARN staff may use the Tribal TANF Turn Around Document (TAD) (FAA-1125A) form to exchange information.
 
NOTE Exchange of information must be faxed, or hand carried by close of business the following workday(g).
 
The TAD is used for the following:
 
To send all change information to either an FAA office or the SRPMIC LEARN office. See SRPMIC LEARN Office for contact information.
When FAA receives change information from the SRPMIC LEARN office, the TAD and change information is date stamped and processed from the date it is received in the FAA office. (See Effecting Changes)
To send FAA applications received or completed at the SRPMIC LEARN office for FAA programs.
 
When the application is received by any of the following means, date stamp and process the FAA programs the date received in the FAA office:
 
Fax
Hand carried
Mailed in
Submitted online (See Health-e-Arizona Plus)
