The Urgent Bulletin emailed on 06/16/2020 reminded staff of the deferral of the EBT Replacement Card Questionnaire (FAA-1578A) form until further notice. This change informs staff that the completion of the FAA-1578A form and the excessive EBT card replacement policy has been reinstated. The UB Banner has been removed. (Effective when an EBT card holder contacts FAA in response to the X152 notice on or after 04/05/2021) Moved policy out of a Warning box and added it as a note.
