Filing an Application - Identifiable Applications
Only a completed or identifiable application form can be accepted for processing. An identifiable application is an application that has all of the following:
●Name of the participant
●Residential and mailing address
●Signature of the participant
The application must be completed by one of the following:
●The participant
●The participant's representative

A completed application is necessary in order to determine eligibility.
