●Complete the FAA0526A Potential Overpayment Referral Unity form in OnBase.
●When the Unity form is unavailable, complete the Potential Overpayment (OP) Referral (FAA-0526A) form from the Document Center. Send the FAA-0526A via email to the Overpayment Unit at [email protected].
