Exception Report Reasons
The exception reasons that may display on the Health‑e‑App Exception Daily Report (MR509) and the AZTECS KC Exception Written Daily Report (MR510), available in SARS, are as follows:
●APPLICATION SIGNATURE DATE IS INVALID
●COUNTY NOT FOUND
●DATE OF BIRTH IS INVALID
●DUPLICATE MSN NUMBERS (There are duplicate Person Sequence Numbers in the same application.)
●FIRST POSITION OF LAST NAME IS HYPHEN (The first character of the last name is a hyphen (e.g. -Smith).)
●GAP IN MEMBER SEQ NUMBER (The Person Sequence Number skips (e.g. 01, 02, 04...).)
●INVALID APP ID
●INVALID RESIDENTIAL ADDRESS
●INVALID SEX CODE
●INVALID SSN
●LAST POSITION OF NAME IS HYPHEN (The last character of the last name is a hyphen (e.g. Smith-).)
●MORE THAN 50 MEMBERS (or MORE THAN 50 CLIENTS)
●NO APP ID FOR (There is no Health-e-Arizona Plus Application ID for the applicant.)
●NO APPLICANT SIGNATURE DATE
●NO DATE OF BIRTH
●NO FIRST NAME
●NO LAST NAME
●NO MEMBER REQUESTED MEDICAL (There is no medical assistance requested for anyone on the Health‑e‑Arizona Plus application.)
●NO MEMBER SEQUENCE NUMBER
●NO NUMBER OF UBS (There are zero unborn when an applicant has been keyed as pregnant.)
●NO PREGNANCY DUE DATE
●NO RESIDENTIAL COUNTY
●NO VALID SITE CODE FOUND (or NO SITE AVAILABLE FOR ZIP)
●NO 01 MEMBER SEQ NUMBER
●NOT A VALID ZIP (or ZIP CODE NOT IN TABLE)
●NUMERIC VALUE IN GIVEN NAME
●NUMERIC VALUE IN SURNAME
●OTHER MEMBER HAD ERROR (or OTHER MEMBER HAS ERROR) (When one applicant has an error, the rest of the applicants on an application all get rejected for the automatic registration.)
●PREGNANCY DUE DATE IS INVALID
●SPECIAL CHARACTERS IN GIVEN NAME (There is a special character in the given name (e.g. #, *, / ).)
●SPECIAL CHARACTERS IN LAST NAME (There is a special character in the last name (e.g. #, *, / ).)
●UBS + MEMBERS (MORE THAN) 50 (There are more than 50 applicants, including unborn.)
●UNDER 10 AND PREG
●DEMOGRAPHIC MISMATCH FOR SSN (The Social Security Number matches on the High Level Client Index, but the applicant name, DOB, or sex does not match the information from Health‑e‑Arizona Plus.)
●MORE THAN 2 ACTIVE CASES
●CLIENT HAS ACTIVE MA
●BAD HLCI CONDITION (An internal system error with the High Level Client Index.)
●MISMATCH 207 VS 154 (The High Level Client Index and Client Basic do not match.)
