To report a document has been uploaded into an incorrect HEAplus account or an incorrect OnBase case, use the
ServiceNow Employee Center (internal use only). Under FAA Case Assistance, select Centralized Document Services (DBME CDS) to complete and submit a new request.
During the implementation of the
Disaster Nutrition Assistance Program (DNAP), applications for regular NA or other program benefits received at the disaster location must be faxed to one of the following:
●(602) 257-7031 (Area codes 602, 480, and 623)
●(844) 680-9840 (Toll Free from any other area code)
NOTE When faxing the applications to CDS, attach the DNAP Case Record Transmittal (FAA-0556A) form.
