The following codes beginning with an A, indicate the error was caused by the agency:

A01	Policy incorrectly applied
A02	Wrong policy applied
A03	Reported information - not acted on
A04	No follow-up on incorrect or incomplete information
A05	Failure to verify required information
A06	Arithmetic computation
A07	Follow-up on anticipated change
A08	Agency – other

The following codes beginning with a C, indicate the error was caused by the participant:

Code	Responsibility Description
C01	Failed to report information
C02	Provided incorrect or incomplete information
C03	Willful misrepresentation
C04	Other participant error

The following codes beginning with an X, indicate the error was not caused by anything that could have been predicted:
