Verification of Burial Plans, Burial Plots and Funeral Agreements
The following verification is required for
burial plans or funeral agreements, or
burial plots:
●Policy owner
●Cash value of prepaid burial plan or funeral agreement
●Name of person the plan, plot, or agreement is for
●Name of funeral home or company
●Plan number
●Availability of cash value to participant
●Availability of equity value to participant
The following are acceptable sources of verification of burial plans, burial plots, or funeral agreements:
●Funeral agreement or burial plan contract
●Funeral Home
●Company that sold plan or agreement
