The
Urgent Bulletin emailed 07/01/2020 informed staff that NA renewals, Mid Approval Contact, and the signature requirements remain suspended through 07/31/2020. The
Urgent Bulletin emailed 10/30/2020 informs staff that the NA/CA renewals due 12/2020 through 05/2021 are extended six months and the Mid Approval Contact (MAC) process due 12/2020 through 06/2021 are suspended.
Effective with all applications registered on or after 07/01/2020, the requirement to complete an interview is reinstated. See

Effective 09/04/2020, see Urgent Bulletin emailed 09/03/2020 regarding FSAD being modified to give a six-month certification period at renewal to all cases that received an extension of their original certification period.
The signature requirement is waived for CA and NA applications dated March 1, 2020 through June 30, 2020, see the
Urgent Bulletin emailed 04/03/2020 and the
Urgent Bulletin emailed 06/02/2020. The
Urgent Bulletin emailed 07/28/2020 informed staff that an extension of the waiver to allow for a verbal attestation in lieu of a signature on the application form is approved through 08/31/2020. The
Urgent Bulletin emailed 08/24/2020 extended that to 9/30/2020. The
Urgent Bulletin emailed 10/09/2020 extended that until further notice.
Only an identifiable
official application form can be accepted for processing.
An identifiable application includes the following:
●Name of the applicant
●Signature of one of the following, as applicable:
Representative (See
Signing the Application for policy and procedures regarding who must sign)
NOTE When an identifiable application is received and no programs are requested, submit the application for all programs in Health-e-Arizona Plus (HEAplus).
A faxed or photocopied identifiable application is acceptable.
When an identifiable paper application is received, complete the following:
●Date stamp the application.
●When the participant is in an FAA Office, copy Page 1 of the identifiable application and give it to participant as proof of application.
●Upload a copy of the application into OnBase as Document Type “FAA0001A” and shred the original. Review the document in OnBase to ensure the signature can be viewed.
NOTE There may be instances where an interview must be completed on the same day an application is received. When TIPS has not automatically registered the CA or NA in AZTECS, FAA staff may be required to manually register the application. The HEAplus Application ID must be correctly keyed on APMA or RERE to prevent the TIPS process from overriding what has been keyed in AZTECS.
The effective date of an identifiable application is the date it is received during FAA
business hours(g). When an application is turned in outside of business hours, the date of application used is the next workday.
All questions on the application that pertain to the programs that are being applied for, must be answered before an eligibility determination can be made.
