Hospital Patient Representative Definition
An employee of a hospital or an agency contracted by the hospital. This assists providers with the following services:
●Offers information on the application process for AHCCCS Health Insurance.
●Locates someone to apply on behalf of an incapacitated patient.
●Assists an applicant with completing an AHCCCS application for health insurance.
●Forwards the application to the designated DES staff or the servicing DES local office.
●Assists DES in obtaining needed verification.
